Contraception

I can still remember the look on my ex's face when we first had unprotected sex, and he just wanted to make sure I was taking the birth-controll-pill. Very nonchalant I answered no.... hihi

Of course I use contraception, like most women, but the pill is a bit outdated if you ask me. So here is a little overview about different kinds of birth-control. And with all these resources unwanted pregnancy is BS!

The birth control-pil (Combined pil)
The combined pill contains two hormones: estrogen and progestin. Most combination pills contain a synthetic estrogen called ethinylestradiol. There are two exceptions: Qlaira and Zoely. The estrogen hormone in Qlaira is called estradiol valerate and is converted in your body into the human estradiol. The hormone estrogen in Zoely is an endogenous hormone, which means: that this hormone is exactly the same as the hormone that your body naturally produces. In addition to the hormone estrogen the combined pill contains also a progestin. The progestin in most pills is derived from the male hormone called testosterone. The progestin in Diane-35 and Zoely is derived from the female hormone progesterone. Progesterone is a substance that you naturally create in the ovaries.

You must take the pill every day at about the same time, in the order indicated on the strip. To reduce the risk of forgetting the pill, it may help to have a regular time to take it. The combination pills-strips contain 21 or 22 tablets, you swallow them for 3 weeks and then you have a stop-week. In the week you stop your menstruation will come, yet you are protected.
There are also pills without stop week, with 28 pills (24 pills plus 4 placebo) per strip, where you during those four days have your withdrawal bleed (= your period).

Advantages of the the combined pill:
Reliable and easy contraception.
With most pills you can skip your period, postpone or shift it.
Certain types of it can be used to treat acne (Diane-35).
If you forget the pill, you can still take it within 24 hours.

Disadvantages of the the combined pill:
Daily peaks and troughs in hormone release.
Chance pill forgotten.
You have to think about you contraception daily.

The estrogen-free pill (Cerazette)
Especially for women who can not tolerate estrogen. This pill does not have a stop week. You take a pill every day throughout the month. If the strip is done, you just start again with a new strip.

Advantages of Cerazette:
Reliable contraception without the hormone estrogen.
No stop week and 12 hours pill forgotten margin.
The only pill that is suitable if you are breastfeeding.
The only pill you can use without objection in migraine with / without aura.

Disadvantages of Cerazette:
You need to think about your contraception daily.
You menstrual pattern may change with use.
You can't choose the timing of your period.
Daily peaks and dips in hormone release.

EVRA: Contraceptive patch
Evra is a combination method and basically the same as the pill. A patch contains 6000 micrograms norelgestromin (progestogen) and 600 micrograms ethinyl estradiol (estrogen) and works for 1 week. This means that per cycle a patch must be stuck three times on the same day of the week as the day you started. It is important that you change the contraceptive patch every week on the same day because Evra is designed to work for 7 days.
Evra may  be pasted at any clean, dry, intact healthy skin piece from buttocks, trunk, upper arm or abdomen. To avoid skin irritation as much as possible it is best to always choose a different location. The patch should be pressed firmly until the edges stick well.
It is resistant to contact with water. Around the patch however, one should not use creams, oils, lotions, powder or make-up because the patch then may detach. It should also not be stuck with an extra ordinary plaster or tape. If the contraceptive patch yet fully or partially is released, you must past a new Evra patch within 24 hours. It is recommended to see every day if the patch is still correctly attached.

Advantages of EVRA:
Reliable contraception.
Gradual hormone release, without daily peaks and valleys.
You can skip your period, postpone or shift.

Disadvantages of EVRA:
Is not reliable if the patch is released.
Around the patch no skin care products.
Is sometimes visible.
Chance of forgetting.

Implanon NXT (contraception swab/rod)
Just like the IUD (see below) Implanon NXT is a very reliable contraception period, which ensures you for 3 years. The swab is invisible (is feel able)  in your upper arm and is designed to continuously give a ggradual, low-dose hormone to your body.

Inserting the swab is done by a doctor and is a short, simple act of about 30 seconds. You just have to bare your arm. After a local anesthetic is the rod will be inserted under the skin of your upper arm using a needle. The removal is done by your GP or gynecologist, fe if you want to become pregnant, or if it is time for replacement after 3 years.

It is important to note that when using the contraceptive swab your period may change or even may stay away. Over 20% of women have no periods after the insertion of the rod, over 30% less often than before and about 20% have more and / or longer periods.

Advantages of Implanon NXT:
Highly reliable contraception.
Three years protection against pregnancy by means of a rod in your arm.
Gradual hormone release, without daily peaks and valleys.
You can quickly be fertile when the rod is removed (from one week).

Main disadvantages of Implanon NXT:
You should let stsaafje insertion and removal by a (trained) doctor.
You menstrual pattern may change after the rod is placed.
You can not control the timing of your menstrual regulation.

Nuvaring (contraceptive-ring)
The vaginal ring has been designed for ease of insertion once a month. Unlike the pill, you not longer have to remember it every day but will keep, just like with the pill, the control about the insertion yourself. With this control you can also use the contraceptive ring to skip, postpone or shift your period (gap week).
You place the ring yourself in your vagina, through the vagina the active substances can be released fast, easy and above all very evenly into your bloodstream. This has several advantages:
  • A low dose of hormones: the contraceptive ring is the combination method with the lowest dose of the hormone estrogen.
  • An even hormone release: the contraceptive ring therefore has no hormone daily peaks such as is the case with the pill.
  • A regular menstrual pattern: with the ring you can expect a regular menstrual pattern.
  • Administration independently of the gastrointestinal tract: the effect of the ring therefore remains reliable after vomiting or diarrhea.
Insertion is as easy as inserting a tampon. Once inserted, the ring changes, according to the contours of the vaginal wall. This way you will have a comfortable location of the ring but also maximum contact with the vaginal wall. The ring is made of soft, flexible material. The design of the ring promotes the ring can not fall out, you will not feel the ring and the ring gives you optimal protection. And if you want to become pregnant again, you can simply remove the ring and you'll soon be fertile.

Disadvantages:
Just because you do not have to remember every day you also have a chance to forgetting it completely.
Chance of fall out / loss.

Injectable medicine:
Depo-Provera (medroxyprogesterone acetate) is an injectable medicine (a “shot”) that prevents pregnancy for up to 3 months with each injection. Depo-Provera is a drug that is very similar to progesterone, a hormone normally produced by the ovaries every month as part of the menstrual cycle. It will be ejected by a docter ever 12 weeks in your buttcheeks or upper arm (Depo-Provera) or in the front of your thigh or lower belly (Sayana).
After a woman stops using Depo-Provera, her normal ovarian function returns after a short time. However, it takes an average of 9 to 10 months to get pregnant after getting the last shot.

Using this as contraceptive is only recommended in women for whom other contraceptives are not an option.
This is because of the unknown long-term effects of boneloss by the use of injections during the critical period for bone growth. Boneloss is greater with a longer use. In general, the bone recovers again - at least partially - after stopping with the injections. It is recommended while using to take enough Calcium and vitamin D.

Important to know is that the use the contraceptive injection can change your period or even make it stay away. Especially if you're new to the contraceptive injection, irregular and longer lasting bleedings can occur. If you continue to use the contraceptive injection for longer than six months reduces the risk of irregular bleeding becomes less and the chance of not getting your period anymore increases.

Advantages of Injections
Reliable contraception.
No more daily reminding your contraception.
No stop week.

Disadvantages of the contraceptive injection
You menstrual pattern may change when using the contraceptive injection.
You can not control the timing of your menstrual regulation.
The injection should be done every three months by a doctor.
Decrease in bone density.
After injection can not be easily removed if you have for example side effects.
It can take up to a year before you get pregnant.

Hormone releasing IUD (Mirena)
An IUD, or intrauterine device, is a small, T-shaped device that is placed into the uterus by a trained healthcare provider during a routine office visit. It provides continuous, highly effective birth control. Keep in mind that no birth control is 100% effective.
Mirena is a hormone-releasing IUD that releases small amounts of a progestin hormone called ´levonorgestrel´ locally into the uterus. Mirena lasts for as long as you want, for up to 5 years.

Mirena is inserted preferably by a trained physician. Before placing the IUD, your doctor will first do an internal pelvic examination, for example to determine the size and location of the uterus and to exclude pregnancy. Placing the IUD then takes a few minutes. In consultation with your doctor, you can get a local anesthetic or take painkillers before the IUD is inserted. Your vagina and cervix get disinfected and with a so-called bullet forceps the uterus mouth gets stretched as much as possible (what can be a little painful). The IUD is then inserted. The insertion may be associated with some pain, blood loss and sometimes fainting. Because the pain upon insertion often is mainly caused by pulling on the uterus, taking a painkiller one hour for the insertion procedure is often more effective than a local anesthetic.

At the end of the Mirena are two thin wires attached which get cut off at the right length after the placement. These threads make it possible to check if the hormone IUD is still in place. You can check this best at the end of your period. Insert a finger into your vagina up to the cervix: if you do and you can feel the thin wires your IUD is in the right place. Do not pull on the wires, this can cause the IUD to come out! When the wires are no longer felt, they can be pushed up into the cervical canal or the Mirena may have shifted. It is also possible that the IUD is located outside of the uterus or completely expelled. In this case see your doctor. Your doctor will then use eg ultrasound to determine this bitchs current location.

After the first period after the placement of the IUD you should be re-examined, were the location and situation of the IUD be checked by your doctor. In order to remove it one only needs to gently pull the wires. But please, always have it done by your doctor, Never pull the wires yourself. You can be fertile very soon after removing the IUD.

Also the use of the hormone spiral can change your menstrual period or even to stay away. 20% of women are in a situation in which their menstruation has slowed or even stopped.

Benefits of Mirena:
Highly reliable contraception.
Five-year protection against pregnancy by means of a spiral in your uterus.
Gradual hormone release, without daily peaks and valleys.
You can quickly be fertile after the IUD is removed.

Disadvantages of Mirena:
You have to get insertion and removal by a (trained) doctor.
Your menstrual pattern may change after the coil is placed.
The insertion can be painful.
You can´t choose the timing of your period.

IUD without hormones (Multiload CU375)
The copper IUD is a T-or horseshoe-shaped piece of plastic with a copper wire around it, placed by a trained doctor through your vagina into the uterus. A copper coil offers like the contraceptive rod and the hormonal IUD for several years protection against pregnancy.
The most sold copper spiral in the Netherlands is called Multiload CU375. This copper coil provides protection against pregnancy for 5 years. Multiload is an effective method without hormones. The favorable effectiveness is partly due to the convenience that you can not forget your contraception. The copper makes the sperm of the male inactive. Rarely, fertilization does take place, however, the IUD then ensures that a possible fertilized egg can not implant in the uterine wall so you still will not get pregnant. Important to know is that the use of an NSAID (anti-inflammatory) can reduce the working of a copper IUD

The insertion is the same as in the other IUD. But remember, because this is a IUD without hormones your menstruation will not change! However, the first and sometimes the second period can come a little earlier than usual and be slightly painful or dure longer. After two or three months your menstrual pattern will be right back to your natural menstrual pattern. The changes in your menstrual pattern also have no effect on the reliability of the copper coil.

Advantages of Multi Load:
Highly reliable contraception.
Several years of protection against pregnancy by means of a spiral in your uterus.
No exposure to hormones.
You can quickly be fertile after the IUD is removed.

Disadvantages of Multi Load:
You have to get insertion and removal by a (trained) doctor.
Your periods may be more painful than what without IUDs.
The insertion can be painful.
You can choose the timing of your period does not regulate.

Sterilization
Sterilization of the man or woman is a permanent form of contraception that are usually only used if you do not want (more) children want. In principle you are permanent infertile after sterilization, still it doesn't give a 100% warranty to never get pregnant anymore, but the chances are very low.
If you want a less definitive solution you could think about the Mirena of Implanon NXT.

Although a sterilization is a good solution for many women, it provides for others not more benefits than any other method of contraception that could suit them well. It is certainly important that you make a deliberate decision in case of sterilization. If you still doubt after the conversation with the doctor/gynecologist or after reading the information given here a sterilization might not be the solution for you, so do not hesitate to take more time to think about it.

Keep in mind none of the above protects you from STD's!
